ABSTRACT

OBJECTIVE@#To observe the therapeutic effect of acupuncture combined with western conventional therapy on type Ⅱ respiratory failure of acute exacerbation of chronic obstructive pulmonary disease (AECOPD) and evaluate the effect of acupuncture on diaphragmatic function and prognosis by bedside ultrasound.@*METHODS@#A total of 111 patients with AECOPD type Ⅱ respiratory failure were randomized into an acupuncture group, a conventional treatment group and a non-acupoint acupuncture group, 37 cases in each one. The routine AECOPD nursing care and treatment with western medicine were provided in the 3 groups. Additionally, in the acupuncture group, acupuncture was applied at Dingchuan (EX-B 1), Feishu (BL 13), Taiyuan (LU 9), Danzhong (CV 17) and Zhongwan (CV 12), etc. In the non-acupoint acupuncture group, acupuncture was given at the points 5 to 10 mm lateral to each of the acupoints selected in the acupuncture group. Acupuncture was given once every day, 30 min each time, consecutively for 10 days in the above two groups. Separately, before treatment, on day 3, 7 and 10 of treatment, arterial partial pressure of oxygen (PaO@*RESULTS@#On day 3, 7 and 10 of treatment, PaO@*CONCLUSION@#Acupuncture as adjunctive therapy achieves significant therapeutic effect on AECOPD type Ⅱ respiratory failure. It improves diaphragmatic function, promotes oxygenation and relieves carbon dioxide retention of artery, alleviates clinical symptoms and reduces the time of mechanic ventilation and hospitalization. Besides, the bedside ultrasound detection can objectively reflect the effect of acupuncture on diaphragmatic function in the patients with AECOPD complicated with typeⅡrespiratory failure.

ABSTRACT

Pai-Nong-San (PNS), a prescription of traditional Chinese medicine, has been used for years to treat abscessation-induced diseases including colitis and colorectal cancer. This study was aimed to investigate the preventive effects and possible protective mechanism of PNS on a colitis-associated colorectal cancer (CAC) mouse model induced by azoxymethane (AOM)/dextran sodium sulfate (DSS). The macroscopic and histopathologic examinations of colon injury and DAI score were observed. The inflammatory indicators of intestinal immunity were determined by immunohistochemistry and immunofluorescence. The high throughput 16S rRNA sequence of gut microbiota in the feces of mice was performed. Western blot was used to investigate the protein expression of the Wnt signaling pathway in colon tissues. PNS improved colon injury, as manifested by the alleviation of hematochezia, decreased DAI score, increased colon length, and reversal of pathological changes. PNS treatment protected against AOM/DSS-induced colon inflammation by regulating the expression of CD4

ABSTRACT

Purpose: This study aimed to evaluate the correlation between infrared thermal imaging-magnetic resonance imaging (MRI)-pathology of microwave ablation (MWA) of lesions in rabbit lung tumors. Materials and Methods: MR-guided MWA was performed in nine VX2 tumor-bearing rabbits. Infrared thermal imaging, postoperative MRI, and pathological presentation were obtained and analyzed. The differences between the infrared thermal imaging-MRI-pathology of MWA were compared. Results: The center of the ablated lesion exhibited a high signal on T1-Vibe, and an isointense envelope was observed; the center of the ablated lesion exhibited a low signal on fat-suppressed turbo spin-echo T2-weighted imaging (TSE-T2WI-FS) and bands of high signal surrounding it compared with before MWA. No statistically significant difference existed between the maximum diameter of the central low-signal area of the ablation zone on TSE-T2WI-FS after MWA, the high-signal area of the ablation zone on T1-Vibe after MWA, and the maximum diameter of the pathological coagulation necrosis area, as well as between the maximum diameter of the isointense signal area peripheral to the ablation zone on T1-Vibe after MWA, the high-signal area peripheral to the ablation zone on TSE-T2WI-FS, the maximum diameter at the 41°C isothermal zone on infrared thermal imaging, and the maximum diameter of the pathological thermal injury zone. Conclusions: MWA of malignant lung tumors had specific MRI characteristics that were comparable with postoperative pathology. Infrared thermal imaging combined with MRI can be used to evaluate the extent of thermal damage to lung VX2 tumors

ABSTRACT

Aim: This study aimed to investigate the technical procedure, safety, and clinical value of the transosseous approach for computed tomography (CT)-guided radioactive 125-iodine (125I) seed implantation for the treatment of thoracic and abdominal lymph node metastases. Subjects and Methods: This was a retrospective study that Nine lymph node metastases in nine patients were treated in our hospital between January 2010 and August 2018. Under CT guidance, at least one puncture path was made through the transosseous approach. The seeds were planted according to the TPS. CT/MRI scans were performed every 2 months after the treatment to evaluate local therapeutic efficacy according to the Response Evaluation Criteria in Solid Tumors. Results: The transosseous approach was successfully established in all patients. The median follow-up time was 11 months (6–36 months). At 2, 4, 6, 8, 10 and 12 months after operation, the objective effective rate and clinical benefit rate were 66.67%, 77.78%, 77.78%, 71.43%, 66.67% and 50.00%; and 88.89%, 88.89%, 88.89%, 71.43%, 66.67% and 50.00%, respectively. The survival rate of the patients at 6, 12, 18, 24, 30 and 36 months after operation was 53.00%, 26.00%, 26.00%, 13.00%, 13.00% and 13.00%, respectively. Conclusions: The transosseous approach for CT-guided radioactive 125I seed implantation was safe, effective, and minimally invasive for the treatment of thoracic and abdominal lymph node metastases

ABSTRACT

OBJECTIVE@#To observe the effect of moxa-cone moxibustion at lung's back-@*METHODS@#Sixty SPF-grade healthy male Balb/c mice were randomly divided into a normal group, a model group, an LY294002 group (LY group), an electroacupuncture (EA) group and a moxibustion group, 12 mice in each group. Asthma model was replicated by using ovalbumin (OVA) sensitization. Except the mice in the normal group, all the mice were intraperitoneally injected with sensitization solution (containing 15 μg of OVA and 30 mg of aluminum hydroxide) on the 1st day, 7th day and 14th day, 0.5 mL per mice; from the 15th day, 1% OVA solution was atomized for 20 min, once a day for 2 weeks; the mice in the normal group was treated with identical operations but with 0.9% sodium chloride solution. The mice in the LY group were treated with injection of LY294002 at tail vein on the 13th day, 14th day and 15th day. At the beginning of the 15th day, The mice in the EA group were treated with EA at "Feishu" (BL 13) and "Zhongfu" (LU 1) with disperse-dense wave, frequency of 2 Hz/20 Hz, intensity of 1 mA, 15 min each time, once a day for 2 weeks. The mice in the moxibustion group was treated with moxa-cone moxibustion at "Feishu" (BL 13) and "Zhongfu" (LU 1) from the 15th day, three moxa-cones per acupoint, once a day for 2 weeks. On the 16th day, 18th day and 22nd day, the incubation period of asthma was recorded. On the 29th day, all the samples were collected. The expressions of IL-17 and IL-10 in serum and bronchoalveolar lavage fluid (BALF) were detected by ELISA method. The pathological changes of lung tissue were observed by HE staining. The percentage of Th17, Treg and Th17/Treg ratio in spleen tissue were detected by flow cytometry method.@*RESULTS@#Compared with the normal group, the incubation period of asthma in the model group was significantly shortened (@*CONCLUSION@#The Th17/Treg is imbalanced in asthmatic body. ABSTRACT

OBJECTIVE@#To establish an individualized Nomogram prediction model for predicting the postoperative recovery of patients with triad of elbow (TE) by analyzing risk factors of triad of elbow joint.@*METHODS@#From January 2012 to December 2018, 116 patients with TE who met the criteria were collected. The independent risk factors were screened by univariate Logistic regression analysis. The statistically significant risk factors were included in the multivariate Logistic regression model. The R software was used to establish the Nomogram diagram model to predict the postoperative recovery of TE patients. C index was used to verify the discrimination, Calibration plot of the model, and the decision curve (decision curve analysis, DCA) to verify the net clinical benefit rate of the model.@*RESULTS@#Forty-four of the 116 patients with TE developed symptoms after operation, with an incidence of 37.93%. Age (@*CONCLUSION@#The Nomogram for predicting postoperative results of TE patients based on six independent risk factors:age, work, smoking, Mason classification of radial head, Regan-Morrey classification of coronal process and immobilization time of elbow joint after operation, has good distinguishing capacity and consistency. Thepredictive model could help clinicians to identify high risk population and establish appropriate intervention strategies.

Objective@#To determine the impact of urination behavior of overactive bladder in female operating room nurses.@*Methods@#A total of 381 female operating room nurses were recruited from three first-class hospitals in Nanjing by convenience sampling method. They were investigated with Chinese version of OAB symptom score scale and Chinese version of urinary behavior scale. The relationship between urination behavior and overactive bladder was analyzed.@*Results@#The total score of overactive bladder female in operating room nurses was 10.05±3.28. There were significant positive correlations between score of urination behavior and score of overactive bladder. Regression analysis showed that the factor scores of urination behavior such as urinary behavior, no urine urination behavior could independently explain 41.3% of the variance for overactive bladder after controlling demographic variables.@*Conclusion@#Clinical managers should focus on the shortcomings of urination behavior of female operating room nurses, and take effective measures to to improve the symptoms of overactive bladder in female operating room nurses.

ABSTRACT

@#AIM: To investigate the correlation between macular morphology and visual acuity and visual deformation in acute central serous chorioretinopathy(CSC). <p>METHODS: A cross-sectional clinical case-control study was conducted in which 38 patients with acute monocular CSC(CSC group)and 38 normal eyes(control group)were included. The best corrected visual acuity(BCVA), slit lamp microscope, indirect ophthalmoscope, frequency domain optical coherence tomography(OCT), color fundus photography and M-charts were performed in all subjects. The BCVA was converted to the minimum resolution angle LogMAR record when doing statistical. The mean LogMAR, macular retinal thickness(CFT), subretinal fluid thickness(SRF), transverse diameter length of subretinal fluid in macular area and visual deformation(M value)were significantly different between the two groups. The correlation between CFT and SRF, transverse diameter length of subretinal fluid in macular area and M value was analyzed. <p>RESULTS: The mean BCVA in acute CSC group was 0.15±0.15(LogMAR), that in control group was 0.12±0.12(LogMAR); CFT was 418.89 ±134.74μm in CSC group and SRF was 286.95±136.77μm in CSC group. The length of subretinal fluid transverse diameter in macular area was 2926.37±1109.66μm. CFT in control group was 217.58±12.49μm, SRF was 0, and the transverse diameter of subretinal fluid in macular area was 0. The M value of CSC group was 0.86±0.40, MH and MV were 0.99±0.38, 0.73±0.43, respectively. Compared with MV, MH was obviously aggravated, the difference was statistically significant(<i>t</i>=4.564, <i>P</i><0.01). There was no correlation between BCVA and the degree of visual deformation in CSC group(<i>r</i>=-0.124,<i> P</i>>0.05). In CSC group, BCVA had no correlation with SRF or the length of subretinal fluid transverse diameter in macular area(<i>r</i>=-0.059, -0.12; <i>P</i>=0.73, 0.48; respectively.)There was a positive correlation between CFT and M value in CSC group(<i>r</i>=0.91, <i>P</i><0.01). The value of MV was positively correlated with the length of transverse diameter of subretinal fluid in macular area(<i>r</i>=0.934, <i>P</i><0.01), and the value of MH was positively correlated with SRF(<i>r</i>=0.949,<i> P</i><0.01). <p>CONCLUSION: In acute CSC group, BCVA had no correlation with CFT, SRF, macular subretinal fluid transverse length and visual deformability, and visual deformability was positively correlated with CFT, SRF and macular subretinal fluid transverse length; MV was positively correlated with subretinal fluid transverse length, and MH was positively correlated with SRF.

ABSTRACT

The role of intestinal microbiota in mammals and humans are gaining increasing attention. The health of piglets requires a dynamically balanced intestinal microbiota. However, there are temporal and spatial changes in the distri-bution and composition of microbiota from the esophagus to the rectum during the life cycle of the pig. The intestinal micro?biota has various beneficial functions in pig, like nutrient metabolism, intestinal mucosal barrier, immune responses and pathogen infection. A variety of factors play an important role in the formation and stabilization of intestinal microbiota, in?cluding the ways of delivery ( vaginal or caesarean) , the diet during infancy ( breast milk or formula feeds) and the usage of antibiotics or antibiotic-like molecules. In this review, we mainly discussed the relationship between intestinal microbi?ota and the intestinal health of piglets from the aspects of the composition and colonization of intestinal microbiota, as well as the functions and influencing factors of intestinal microbiota, so as to further understanding the importance of intestinal microbiota in intestinal function of piglets.

ABSTRACT

Objective To analyze the correlation between pathological of non small cell lung cancer(NSCLC)and mutations of the epidermal growth factor receptor(EGFR)gene.Methods Selected 96 patients with lung cancer for EGFR mutation de-tection in Shaanxi Provincial People's Hospital from January 2014 and April 2015,comparison of different types of NSCLC EGFR mutation rate.Results Gene mutation sensitive in 24 cases,in 12 cases and location of the mutation L858R,10 cases of mutation at position 19DEL,1 cases of mutation position G719X and S768I,1 cases EML4-ALK fusion.Mutation rate of 29.17%.Slightly lower than the international and domestic NSCLC epidermal growth factor receptor gene mutation sensitive rate of 30% of the average.Conclusion Target to treatment is aimed at the effective means for the treatment of NSCLC pa-tients with individualized,has reliable efficacy,less toxicity and adverse reactions,will become one of the most promising treatment,the study results below average,less attention to the base of the study population and need to raise awareness of routine testing.

ABSTRACT

<p><b>OBJECTIVE</b>To explore the clinical effect between two approaches for the treatment of supracondylar fractures of humerus by K-wires in children.</p><p><b>METHODS</b>From Jan. 2004 to Dec. 2006,117 children with supracondylar fractures of humerus were divided into two groups according to different methods of the K-wires fixation involving two K-wires group and three K-wires group. In two K-wires groups,there were 45 children including 31 boys and 14 girls with an average age of 5.6 years ranging from 1 to 11 years; according to Garland classification, 19 cases were type II, 26 cases were type III. In three K-wires groups, there were 72 children including 47 boys and 25 girls with an average age of 6.8 years ranging from 2 to 12 years; according to Garland classification, 22 cases were type II, 50 cases were type II. The movement and carrying angle of elbow joint were measured for all these patients. According to Flynn criteria for supracondylar fracture the results of two approaches with K-wires were compared.</p><p><b>RESULTS</b>All patients were followed-up from 2 to 24 months (means 15.4 months). In two K-wires group, the results of Flynn were excellent in 27 cases, good in 12 cases, fair in 4 cases and poor in 2 cases, 41 cases gained bony healing, however, fail of fixation arosed in 4 cases and cubitus varus arosed in 2 cases. In three K-wires group, the results of Flynn were excellent in 60 cases, good in 11 cases and poor in 1 case, all cases of three K-wires group achieved bony healing after 6 weeks of operation, the function of joints recovered through exercise and cubitus varus arosed in one case after operation.</p><p><b>CONCLUSION</b>The technique of fixation with K-wires is a stable and reliable methods for unstable supracondylar fracture but medial-lateral three-pin fixation is better than one with two pins.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To determine the clinical features and therapeutic strategy of humeral supracondylar fractures with nerve injuries in children.</p><p><b>METHODS</b>From June 2000 to December 2006,28 patients of humeral supracondylar fracture (Gartlang III) with never injuries were analyzed. Twenty-four patients were close injuries and the others were open. Sixteen cases (group A)were treated with close reduction and K-wires fixation. Twelve cases (group B)were treated with open reduction and nerve exploration, among them, contusive nerve injuries in 8 cases, incarceration in 3, complete neurotmesis in 1.</p><p><b>RESULTS</b>The symptoms of nerve injuries of 28 cases recovered completely within 4 to 6 weeks in 13 cases, 8 to 10 weeks in 2 cases and 12 weeks in 1 case in group A, meanwhile, within 4 to 6 weeks in 6 cases, 8 to 10 weeks in 3 cases and 12 to 16 weeks in 3 cases in group B.</p><p><b>CONCLUSION</b>Most nerve injuries in humeral supracondylar fractures in children can be cured after close reduction of the fracture. Sometimes, open reduction and nerve exploration are necessary, which has important practical significance.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To analyze the reasons on complications of treatment with elastic nail in children's long bone fracture.</p><p><b>METHODS</b>Sixty-six cases (75 parts of long bone fratures) were treated by elastic nail including 49 male and 17 female. The age ranged from 3 to 17 years, mean 7.8 years. There were 35 femur fractures (2 cases were hibateral), 20 tibia and fibula fractures (12 cases were tibia fractures), 8 radial fractures (1 case was ulna fracture) and 3 humerus fractures. The cases included 4 open fractures and 62 closed fractures. All cases were fresh fractures, no multi-segmental fractures. Three cases associated with brain and chest injuries. These cases were treated by open or closed reduction and internal fixaion with elastic nail. A cast or brace had been used after operation for a month. Following-up included the function of the joint,the bottom of the nail and the callus. Complications were timely recorded.</p><p><b>RESULTS</b>All the patients were followed-up for 12 to 29 months, averaged 17 months. The cases occurrenced compilications including 2 cases of nonunion, 2 of new fracture, 1 of displacment, 4 of joint dysfunction, 3 of irritation of the bottom of the nail and 1 malunion.</p><p><b>CONCLUSION</b>Strict indication, well design,canonical operation is a good way to avoid compliacations. At the same time,early treatment can reduce the sequela.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To expore the prevention of ulnar nerve injury during fixation of supracondylar fracture in children by a medial-lateral three-pin fixation technique.</p><p><b>METHODS</b>Eighty-one patients including 54 boys and 27 girls with average age of 6 years old (2.5 to 11 years)were treated by a three-pin fixation technique with insertion of two Kirscher wires from the lateral side and the third wire through the medial side. They were followed-up without any complications related to the ulnar nerve. According to Garland system, there were 25 cases of type II supracondylar fracture and 56 of type III. All patients were treated by a medial-lateral three-pin fixation technique, and external fixation with plaster for 3 weeks.</p><p><b>RESULTS</b>All cases achieved immediate stability and long-term bony fusion postoperatively and no iatrogenic ulnar nerve injury happen.</p><p><b>CONCLUSION</b>The technique is a stable and reliable methods for unstable supracondylar fracture and the iatrogenic injury ulnar nerve can be avoided completely.</p>

Objective To investigate the causes,diagnostic methods,prevention and cure of postmenopausal bleeding(PMB).Methods The clinical and pathological data of 210 PMB patients admitted in this hospital were retrospectively analyzed.Results The causes for PMB were benign diseases(69.5%),the most of which were infec- tious diseases(49.5%),non-organic disease(20.5%)and malignant tumor(10.0%).Compared with benign diseases and non-organic diseases,the attack age of malignant tumor was higher[(67.1?6.9)years old],menopausal dura- tion was longer(11.6?2.9)years,bleeding duration was longer[(157.6?26.4)days],and mucous membrane was obviously thickened[(14.6?3.2)mm](P0.05).Conclusion The main cause for PMB is benign diseases,followed by non-organic diseases.More attentions should be paid to the PMB patients with higher attack age,longer menopausal duration and bleeding dura- tion,and obviously thickened mucous membrane,so as to be precautions to the occurrence of malignant tumor.

ABSTRACT

To detect the epidemiological factors of Clonorchis sinensis infection in Hengxian County, one of counties in Guangxi Zhuang Autonomous Region where Schistosoma japonicum was endemic but eliminated in the late 1980s, a questionnaire was designed with 37 questions covering socioeconomic conditions and human behavior, contamination of the environment and fish ponds, inadequate farming/fishery practices; and the formalin-ether sedimentation technique was used to examine the feces of cats, dogs and pigs for the eggs of C. sinensis. Fresh fish was sliced and digested to detect the metacercariae of C. sinensis under a stereomicroscope. Of 1,521 people interviewed, 64% of the interviewees did not know about fluke disease or its transmission route, 46% of those who knew about the fluke believed that the infection caused no harm or only slight harm to their health. More than half of the interviewees (51%) ate raw fish at least 1-2 times per month, more among the middle-aged males. Eight percent of them used the same utensils for both raw fish and cooked food. When advice was given not to eat raw fish, 73% of the interviewees thought it was not feasible. In relation to pisciculture, 25% and 9% of the owners of fish ponds fed their fish with feces of domestic animals and human feces, respectively. The prevalences of C. sinensis infection in cats, dogs and pigs were 70, 50 and 27%, respectively, and the infection rate in fish was 40%. These results indicate the poor knowledge, and beliefs, and unhealthy behaviors in the human population. They also show poor environmental hygiene and inappropriate farming/fishery practices are important in the increase of C. sinensis prevalence in humans. Combined interventions, including health education, environmental modification, reform of traditional farming/fishery practices, mass screening and chemotherapy for humans, and the management of domestic animals are needed in developing control strategies to decrease C. sinensis infection in the human population.
